The tragedy of Indonesia AirAsia Flight 8501

On December 28, 2014, the Indonesia AirAsia Flight 8501 was scheduled to fly from Surabaya in Indonesia’s Java to Singapore. As per the schedule, it had to depart from the Juanda International Airport and arrive at Singapore Changi Airport. On the fixed time, the flight took off and reached its cruising altitude 14 minutes later at 9,800 metres. It took air route M635 and headed over the Western Java Sea. The Jakarta Air Traffic Control (ATC) maintained contact with the plane.

Around 25 minutes later, a caution was aired in the flight which indicated that there was a fault with the rudder system. Captain Iriyanto, after reading how to fix the failure, rebooted the aircraft’s two Flight Augmentation Computers and when the same problem took place around nine minutes later, the captain fixed the problem in the same way.

A few minutes later, the flight requested Jakarta ATC to climb to a higher altitude to avoid inclement weather and it was asked to wait. Meanwhile, for a third time, the rudder limiter problem took place and the captain fixed it in the same way like the previous two times. When the problem occurred for a fourth time, he fixed it in a different way which he saw being performed by a ground engineer and thought that it could be done in flight. It changed the flight system and then, First Officer Plesel over-corrected the fault twice. The flight climbed to 38,500 ft before descending at a rapid pace. It crashed into the Java Sea, resulting in the destruction of the plane.

There were 162 people on board the plane and all were killed in the crash. There were 155 passengers on board whose details AirAsia released later. The passengers included 17 children, 137 adults and there was one infant among them. The crew had two pilots, four flight attendants and an engineer.
